June 12, 2008 (Current Date) Dear Mr./Ms. Employee XYZ: (Employees First and Last Name) (Mr/Ms Name) you are being issued a formal written warning for Tardiness. You acknowledged that you were aware of our company policy on Tardiness at your orientation on March 3, 2008. You also signed acknowledging that you received a copy of the company employee handbook on that same date which clearly outlines this policy. (NOTE - It is critical that you have an employee handbook and they sign acknowledging that they received it.) Specifically, on November, 8th you were tardy 8 minutes, 9th you were tardy 6 minutes, and 10th you were tardy 7 minutes. Need to Write an Employee Handbook?
You are to report work on time and ready to work at the beginning of your scheduled shift. Future violations of any company policy may result in additional disciplinary action, up to and including, possible termination.
